Request a replacement online through the province’s Manitoba Health Card site, or call the Insured Benefits Branch at 204-786-7101. Your health numbers do not change when you get a new card, and your current coverage stays valid while you wait.

A lost, stolen or damaged Manitoba Health card is replaced by Manitoba Health, Seniors and Long-Term Care, through its Insured Benefits Branch. Nothing about your coverage changes: the province says your Personal Health Identification Number (PHIN) and your registration number stay the same on a new card, and your existing card remains valid.
If you know your registration number and your PHIN, you can request a replacement using the province’s online form. The request is made through your “My Manitoba Health Card” account, where you choose a plastic card, a digital card, or both.
If you do not have those numbers, the province offers a paper Replacement Health Card Form instead. Forms must be signed in ink or with a verified digital signature.

No computer at home? Manitoba lists public access computers at the Bilingual Service Centre, main floor, 170 Goulet Street in Winnipeg, and at municipal offices in rural areas.
Call the Insured Benefits Branch at 204-786-7101, or toll free at 1-800-392-1207. Business hours are Monday to Friday, 8:30 AM to 4:30 PM. The office is at 300 Carlton Street, Winnipeg.
This catches many families out. The province only accepts changes from the cardholder, or from a person holding their Power of Attorney, a Substitute Decision Maker, or a Committeeship. Those documents must already be on file with Manitoba Health.
If you are arranging care for a parent and your documents are not on file yet, call 204-786-7101 and ask about the document process before you need the card. It is far easier to sort out now than on a day when your parent is unwell. Our caregiver guide covers the other things worth doing early.
Manitoba now offers a digital health card that lives in the province’s MB Wallet app, available from the App Store and Google Play. You request it through the same “My Manitoba Health Card” account by choosing a replacement card and selecting “Digital”. The province says it may take up to two hours for the digital card to become available.
Be aware of scams. Manitoba Health, Seniors and Long-Term Care says it will never ask for your personal or financial information through an unsolicited text message. If a message about your health card looks unusual, contact the Insured Benefits Branch directly.
Yes. Manitoba began issuing new plastic health cards on January 15, 2025, and says existing paper cards continue to be accepted at health care facilities. Family health cards are no longer printed, but existing ones remain valid. A replacement request now results in an individual card for each person.
The plastic card carries your name, your registration number, your PHIN, your birthdate and your official language, plus a QR code. The province says the QR code only holds the information printed on the card and does not give access to any medical records.
The province publishes its service standards and current processing dates on the Manitoba Health Card and Coverage page, and updates them regularly. Check there for today’s timing rather than relying on a figure that may have changed.
Manitoba requires you to report changes such as a new address, a change of name, a marriage, a birth, a divorce or separation, or a death in the family within 30 days to remain eligible for coverage. If your card was lost during a move, update your address at the same time as you request the replacement.
Your coverage does not stop because a card is lost. What matters is that your registration is valid. When you book with Doctors To You, you give us your Manitoba Health card number and we confirm coverage before the visit. If your card is lost, damaged or you are unsure of your numbers, tell us when you call. It is a common situation, and we will explain your options before anything is provided.
